Our FCP service is managed by Quay Healthcare CIC, the GP Federation.
First Contact Physiotherapists (FCPs) are qualified autonomous clinical practitioners who can assess, diagnose, treat and manage musculoskeletal (MSK) problems and undifferentiated conditions.
They can help patients with musculoskeletal issues such as back, neck and joint pain by:
- assessing and diagnosing issues
- giving expert advice on how best to manage their conditions
- referring them onto specialist services if necessary.
Patients with back and joint pain, including conditions such as arthritis, will now be able to contact their local physiotherapist directly without a referral, or contact your GP practice directly to request an appointment.

A frailty Care Coordinator holistically supports patients with mild to severe frailty within the community. They collaborate with patients to create a patient-centred care plan and provide access to support services within the local area that are most appropriate for you.
East Warrington PCN's Frailty Coordinator is called Dionne. Dionne's services are available to any patients registered at Padgate , Fearnhead Cross or Birchwood Medical Centres. To book an appointment , please contact your GP surgery as normal and ask to for an appointment with the Frailty Care Coordinator.
East Warrington PCN are currently staffed with two Mental Health Practitioners, Sue and Sam, who support adults with mental health issues to live well in the community.
This role in particular has been created to enable professionals with mental health expertise to be based in GP surgeries or neighbourhood settings, to support people with complex mental health needs and act as a ‘bridge’ between primary care and specialist mental health services.
Mental Health Practitoners can offer support with the following :
- Anxiety
- Depression
- Bipolar
- Psychosis
- Eating disorders
- Personality Disorders
- and more.

A care coordinator provides coordination and navigation for patients across a varity of heath and care services , working closely with other members of the PCN team and other health care professionals.
Kista, East Warrington PCN's Care coordinator, focuses her work on children and their families, helping to understand the child needs and achieve better healthcare outcomes by actively signposting and providing information support. Some examples of the issues Kista can support with are :
- Neurodiversity
- Challenging behaviour
- Friendships and bullying
- Eating habits
- Struggling to attend school
- Speech and language delay
Besides children, young people and their families, Kista also supports newly diagnosed cancer patients by actively signposting them and their carers to services that may benefit them, such as the Macmillan Cancer Support service.
