Social Prescribing Link Workers take a holistic approach to people's health and wellbeing. They can connect you to community groups and other services for practical and emotioanl support. They can spend time with you to talk about what matters most to you, and support you to find an alternative to medication, or support which can help alongside.
The service works for a wide range of people, including those:
- With one or more long term condition
- Who are lonely or isolated
A social prescriber can connect you to local services to help with:
- Housing & benefits
- Financial support & advice
- Bereavement
- Carers support
- Social / support groups / befriending
- Lifestyle support such as walking groups, exercise and smoking cessation
- Volunteering opportunities
This service is provided by LiveWire Warrington for all patients across East Warrington Primary Care Network.
If you feel like you would benefit from an appointment with Jenny (our Social Prescriber) please contact your surgery as you usually would who will be able to make a referral into the service.
